Dear Parent/Guardian,
This message is to invite you to let your child do the About You survey, and provide you with information regarding the project. Please find attached a copy of the Parent/Guardian Letter and Consent Form and the Student Information Letter. Please read the information carefully so you are fully informed about the survey.
Could you please give your child the attached Student Information Letter as the survey is voluntary and if he or she does not wish to take part they do not have to. They also have the option not to answer individual questions within the survey.
We are asking you for your consent to have your child do the About You survey. If you do not want your child to do the survey you must either:
- Return the Opt Out Refusal of Consent Form included, to your child’s school by the day before the 13 September 2018 It is important that we receive the form before to visiting the school.
Or
- Opt out via email, If you do not wish for your child to do the survey, please send an email by the day before the 13 September 2018 stating that you wish to opt out of your child’s participation, along with their name, school and year level to: aboutyou@colmarbrunton.com and we will ensure that they do not participate in the survey on the day.
If we do not receive a Refusal of Consent Form from yourself via hard copy or email before the visit date mentioned within this letter it means that you give your consent for your child to do the About You survey.
