Please find attached a copy of the Parent\/Guardian Letter and Consent Form and the Student Information Letter. Please read the information carefully so you are fully informed about the survey.<\/p>\n<p>Could you please give your child the attached Student Information Letter as the survey is voluntary and if he or she does not wish to take part they do not have to. They also have the option not to answer individual questions within the survey.<\/p>\n<p>We are asking you for your consent to have your child do the About You survey.
